Duloxetine (Cymbalta) is approved in Japan for depression, diabetic neuropathic pain, fibromyalgia, chronic low back pain and osteoarthritis pain, so both psychiatrists and orthopedic clinics prescribe it. Capsules are 20 mg and 30 mg with many generics; it is not a controlled substance.
