Opioid (cough, pain)

Codeine (Tylenol #3, co-codamol)

Codeine phosphate / dihydrocodeine · ジヒドロコデインリン酸塩

Codeine sits on both sides of Japan's rules. Products with more than 1% codeine or dihydrocodeine, including Tylenol #3 and most co-codamol, are narcotics and need an import permit before you arrive. Weak dihydrocodeine cough syrups and cold tablets are sold OTC in Japan, so mild cough relief is easy to find locally.

Over the counter

Low-strength dihydrocodeine (1% or less) is in many Japanese OTC cough and cold remedies (Designated Class 2), limited to one pack per customer. Anything stronger is a narcotic.

Frequently asked questions (FAQ)

Can I bring Tylenol #3 or co-codamol into Japan?

Only with a narcotics import permit obtained in advance from the Narcotics Control Department (allow 2–3 weeks). Without it the medicine is confiscated. For short trips, most travelers switch to plain acetaminophen or an NSAID.
